Please fill out the application form (coming soon) found on this website and submit the following documents to
- Curriculum vitae
- Photocopy of diploma (note that the CONB reserves the right to see the original diploma)
- Photocopy of Birth Certificate, passports, permanent resident card, work permits visa
- Photocopy of professional liability insurance policy
- Complete Proceeding History Form (police records check) and Vulnerable Sector Check
- An application fee of $50 is to be paid to the CONB.
Membership obligations
The CONB assures that all new and existent members are in good standing with the requirements of the college. Every member must submit to the CONB:
- Membership fees ($1,500) due no later then September 1st of each year. Penalty fees will apply in the event of late payment.
- Proof of renewal of professional coverage (every year)
- Complete Proceeding History Form (police records check)
- Certificates of continuous education (50 credits every 3 years)
- Emergency First Aid + CPR Level C with AED
- *Any member that do not provide these to the CONB will be at risk of loosing their membership with the college.
